TYPES OF CERTIFICATION CERTIFICATES AND FEES

Five types of certificates are issued by CMS. All are valid for 2 years.

1. CERTIFICATE OF WAIVER: May only perform waived tests. Fee = $150.00.

2. CERTIFICATE FOR PHYSICIAN PERFORMED MICROSCOPY: May perform tests in this category and those in the waiver category. Fee = $150.00.

3. REGISTRATION CERTIFICATE: This certificate will be issued to a lab until CMS determines if all requirements are met through on-site inspection or verification of accreditation. The fee for this certificate includes the inspection fee and varies by number of specialties and number of tests performed. Fees can range from $350.00 to over $3,000. Laboratories have the option to be inspected privately through COLA, which charges by number of physician and specialties only. Fee for COLA inspections range from $900 up.
