CERTIFICATE

This certificate will be issued to labs performing test of moderate or high complexity that were previously unregulated subsequent to determination. This Certificate will be valid until a determination of compliance is made. Inspection will be required. Fees for this certificate (good for two years) will range from $300.00 up - depending on amount of testing being done and number of specialties.
